POSITION SUMMARY: 
This position provides basic care for residents/tenants to ensure the highest degree of quality care is maintained at all times within a safe environment; assists with activities of daily living, offers emotional and social support, and attends residents’/tenants’ overall physical comfort. 

This position is available for Full-Time (PM and NOC shifts). 

PM (B Shift) is 2:00pm - 10:30pm, and Overnight/NOC shift is 10:00pm - 6:30am. 

ESSENTIAL FUNCTIONS:  
The following duties are normal for this position. These are not to be construed as exclusive or all-inclusive. To perform this job successfully, an individual must be able to perform each duty satisfactorily. Other duties may be required and assigned. 

  • Provides personal hygiene including bathing (tub, shower, bed bath), oral care, skin and nail care and hand washing.
  • Completes resident grooming including hair care, shaving, dressing and weighing. Encourages residents/tenants toward independent personal care.
  • Assists with toileting including bowel and bladder retraining and perinea care.
  • Performs and assists with mobility, transferring, ambulation, positioning and range of motion per the care plan.
  • Assists in orienting residents/tenants upon admission and throughout stay as needed.
  • Documents accurately all care given, including dietary intake and output, temperature, weight, blood pressure pulse and respiration, using approved abbreviations and descriptive terms.
  • Reports to nurse on units for written and verbal assignments which include daily use of care plans for assigned residents/tenants before initiating care.
  • Participate in the planning of resident care through shift reports and resident care staffing meetings when assigned.
  • Informs verbally and/or submit in writing any observations and changes in a resident for the licensed nurse to evaluate including dietary intake.
  • Serves residents/tenants their meals; motivates and assists them with intake of nourishment and fluids.
  • Answer all call signals and requests of residents/tenants immediately.
  • Encourages/assist residents/tenants in keeping their rooms clean and orderly; assists with making beds.
  • Maintains nursing unit in a clean and orderly manner.
  • Keeps all residents’/tenants’ personal belongings and equipment clean.
  • Escorts residents/tenants to appointments and/or therapy departments as scheduled.
  • Participates in unit activity programs planned for residents/tenants as needed.
  • Attends and participates in unit nursing staff meetings and mandatory in-services.
  • Performs a total inventory of all resident’s possessions on admission, discharge or transfer.
  • Ensure all residents/tenants’ possessions are properly labeled.
  • Responds to resident emergencies including bleeding, falls, seizures and performance of Heimlich maneuver.
  • Assists with post-mortem care as assigned.
  • Transports dietary carts and linen carts.
  • Other duties as assigned.

KNOWLEDGE, SKILLS AND ABILITIES

  • Knowledge of the principles, standards, practices and techniques of skilled nursing.
  • Knowledge of the dangers inherent in and precautions to be taken in the use of drugs and other medication.
  • Knowledge of local, state and County rules, regulations, procedures and policies pertaining to nursing services.
  • Ability to accurately and timely document resident care, services provided, and observations in the electronic medical record. 
  • Technical proficiency in the use of relevant software applications, including electronic records systems, databases, standard office equipment, and other technology tools relevant to the position.
  • Knowledge of personal protective equipment (PPE) and its proper use to promote personal and resident safety.
  • Ability to maintain confidentiality of all resident care information and protect resident rights and privacy in accordance with applicable laws, regulations, and organizational policies
  • Ability to create and maintain an atmosphere of warmth, respect, and personal interest, as well as a clean environment.
  • Ability to communicate effectively and interact effectively, respectfully, and tactfully with residents, families, the public and staff. 
  • Ability to organize and prioritize assigned duties, respond appropriately to changing resident needs, and complete assigned responsibilities in a timely manner with a high degree of accuracy and attention to detail. 
  • Ability to work effectively as part of a resident care team and communicate and collaborate with nursing staff, coworkers, and other members of the organization.

PHYSICAL REQUIREMENTS 

  • Work may involve the occasional exertion of up to 75 pounds of force; work regularly involves sitting, frequently involves standing and performing fine motor skills and repetitive motions, and frequently involves walking, stooping, kneeling, crouching or crawling and reaching above shoulder level or below the waist.
  • Visual ability to perform tasks such as reading documents, operating vehicles, using computer screens, or working with detailed data.
  • Ability to communicate and exchange information effectively with others, with or without reasonable accommodation.
  • Work requires observing general surroundings and activities to safely perform the essential job functions. 

WORK ENVIRONMENT 

  • Work is primarily in a health care setting and may require work in any area of the facility.
  • Work frequently requires exposure to bloodborne pathogens and may be required to wear specialized personal protective equipment (PPE).
  • May be subject to exposure to infectious wastes, diseases, and medical conditions.
  • Vaccinations and testing as required by local, state, or federal regulations and/or other regulating entities.
  • Ability to work the allocated hours of the position. 

MINIMUM QUALIFICATIONS:

EDUCATION AND EXPERIENCE REQUIREMENTS 

Certified Nursing Assistant – Nursing Home 

  • Age 16 or older.
  • Wisconsin certification as a Certified Nursing Assistant and on state registry.
  • Must successfully pass criminal and caregiver background check.
Starting pay is determined within the expected hiring range based on qualifications, directly related experience, and internal equity. Employees may advance through the full salary range in accordance with County policy. 
 
Expected Hiring Range: $21.87 - $24.74/hour
Full Salary Range: $21.87 - $30.46/hour
