Case Manager/ Social Worker - Children’s Developmental Services Worker

This position is primarily responsible for providing support and service coordination for the Birth to 3 program. This position also has some responsibility for support and service coordination for Children’s Long Term Support program. This includes the following types of services: developmental screening/assessments, plan development, case planning, monitor/coordinate service delivery, and child find activities in accordance with WI state guidelines.

Qualifications: Minimum of actively working to achieve a Bachelor’s degree from an accredited college or university in a Human Services related field. Preferred Bachelor’s degree from an accredited college or university in a Human Services or education related field and credentialed as a Certified Social Worker (CSW, CAPSW, CISW, or LCSW) in the State of Wisconsin
