Are you passionate about helping others and making a difference in your community? We’re hiring a Community Health Navigator (CHN) to connect residents with the health and social resources they need to thrive.

What You’ll Do:
•  Build trusting relationships with community members
•  Provide health education and link people to resources
•  Advocate for access to basic needs and services
•  Collaborate with healthcare and community partners

What We’re Looking For:
•  Background in health or social services (degree preferred, experience accepted)
•  Compassionate, resourceful, and great with people
•  Strong communication and organization skills
