SUMMARY Provides the general public, but particularly adults who are older or have a disability, with information and assistance on a wide range of community resources; informs and educates people about long term care options; assists in connecting them to programs and services, including public and privately funded options. Administers the Wisconsin Adult Long Term Care Functional Screen (LTCFS) for functional eligibility determinations for community-based publicly funded long term care. Counsels and educates consumers seeking community-based care on financial eligibility requirements. Performs enrollment functions for community-based long term care.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
Determines through in-person or telephone interviews or home visits the nature of the issues and concerns and range of services needed for individuals and families seeking resources. Gathers sufficient information to accurately identify and clarify inquirer's problems and needs to determine recommendations for short- and long-term solutions. Assists consumers in locating and utilizing private pay or community resources.
Provides accurate and timely completion of contact documentation within procedures and time frames as required by the Federal and State Government and also within ADRC guidelines and procedures.
Conforms to Federal, State, and ADRC policies and procedures regarding professional conduct and service delivery to ensure the health and safety of those served by the agency and community members.
Maintains a working knowledge of community resources and is able to communicate available resources to consumers in a manner understandable to the consumer. If appropriate, assists individual in connecting with a resource if they are unable; provides advocacy.
Provides short-term service coordination according to ADRC policy.
Provides follow up as needed to determine outcomes and provides additional assistance in locating resources and arranging services.
ESSENTIAL DUTIES AND RESPONSIBILITIES (continued):
Performs the LTCFS and provides information about publicly funded long-term care and assists consumers throughout the eligibility determination and enrollment process. Responsible for timely and accurate entry to State computer systems of LTCFS’s, enrollment, and disenrollment from publicly funded community-based long term care programs. Provides disenrollment counseling to individuals choosing to leave a long-term care program.
Assists individual in completing Medicaid application and other public benefit programs (e.g., Foodshare) if needed. Monitors application progress and communicates with the Income Maintenance Consortium as necessary.
Participates in various transition activities designed to help youth who have a disability transition from school to the adult service system.
Participates in program development, marketing, and outreach activities including representing the ADRC at public information events.
Performs various assessments for ADRC programs and services to meet Federal and State reporting requirements. Recommends eligibility for services to management.
Participate in the Oneida County Emergency Management Plan as needed and directed by the plan.
DOCUMENTATION AND QUALITY ASSURANCE
Maintains accurate, timely and complete documentation.
Participates in prevention activities, support groups, classes and other initiatives.
Participates in annual Quality Assurance/Quality Improvement projects and activities.
Arranges work schedule as necessary or directed to meet the program and consumer service needs.
Represents the ADRC to the community at large through professional interaction, public speaking, media presentations, and participation in community advisory groups as requested.
Participates in staff meetings and training activities.
Complies with applicable federal and state laws, administrative rules, established agency procedures and accepted professional standards.
Participates in on-going training, maintaining current knowledge to ensure compliance with federal and state regulations.
Maintains the confidentiality of client information as required by State and Federal laws and regulations and professional practice standards.

QUALIFICATIONS (continued):

Knowledge of the principles and practices pertaining to social work.
Knowledge of laws, regulations and practices pertaining to social service programs.
Ability to proficiently use a computer and related office equipment; demonstrating familiarity with required software and database programs.
EDUCATION and/or EXPERIENCE
Four year college degree, preferably in social work or a related field.
Three to five years Social Services/Human Services experience is strongly preferred.
Other significant and applicable experience will be considered.
LANGUAGE SKILLS
Ability to communicate effectively, both orally and in writing, with various types of people, both individually and in groups.
MATHEMATICAL SKILLS
Good mathematical skills.
Must be knowledgeable of business math.
REASONING ABILITY
Ability to understand complex situations and interactions and tailor solutions to unique circumstances.
Ability to deal tactfully with difficult situations.
Ability to establish priorities for service intervention.
CERTIFICATES, LICENSES, REGISTRATIONS
Certified as a Wisconsin Adult Long Term Functional Screener. Required within three months of hire.
Certified Social Worker preferred.
Valid Wisconsin's Driver License
