POSITION SUMMARY:
This is position is responsible for providing programmatic screening, including information, assistance, and referral services to older adults and adults with disabilities inquiring about community and division resources.  This position performs a variety of duties including triaging consumer contacts to increase efficiencies, providing support for ADRC professional staff and the ADRC Council on Aging and Disabilities, and providing short-term case management duties and/or other division-related tasks under the direction of the ADRC professional staff.  

ESSENTIAL FUNCTIONS:

  • Provides customer-driven prompt, accurate and friendly customer service responses to requests for information about ADRC benefits and services and access to long term care resources to individuals age 60 and over and adults with disabilities, their families, caregivers, and the public primarily on the telephone, in person, and through e-mail communications.
  • Assesses needs, acquires necessary information, refers to ADRC staff and other service providers, and follows up as needed.
  • Provides information and referral assistance for ADRC customers inquiring about applying for public benefits, private benefits, long-term care options, counseling and other community resources.
  • Assists ADRC customers with completing applications for Medicaid and Medicaid-related programs including assisting with gathering necessary documentation, tracking progress of applications, and monitoring department performance management measures for Medicaid applications.
  • Conducts home visits, as necessary, for gathering documentation.
  • Serves as the department liaison with the economic support consortium (Great Rivers).
  • Assists Benefit Specialists with the annual open enrollment period for Medicare.
  • Assesses the needs of the customer, makes referrals to outside agencies and/or schedules appointments.
  • Documents all customer contacts and other program information in PeerPlace database.
  • Identifies and resolves duplicate entries.
  • Maintains up to date knowledge on State and department reporting requirements.
  • Participates in ongoing training.
  • Develops and maintains ADRC resource information.
  • Organizes, compiles and edits monthly ADRC newspaper in publish ready format.
  • Completes 100% time reporting.
  • Other duties as assigned.

WORK ENVIRONMENT

  • Work is primarily in an office setting but may also be in consumers’ homes, school, residential facilities, work sites, other public settings, etc.

MINIMUM QUALIFICATIONS:

EDUCATION AND/OR EXPERIENCE REQUIREMENTS

  • Associate degree in health or human services-related discipline, business administration, communication, or a related field AND a waiver from the State of Wisconsin Department of Health Services OR a Bachelor’s degree in health and human services-related discipline.
  • One (1) year experience working with one or more of the consumer populations served by the ADRC.
  • Must successfully pass criminal and caregiver background check.
Full Pay Range: $22.29 - $31.05
Expected Pay Range: $22.29 - $25.22
